24 Panel Drug Test
A 24 panel drug test cup screens one urine sample for twenty-four drugs,
with results in about 5 minutes. It starts with the drugs on a classic 12 panel drug test
and adds newer threats like xylazine, medetomidine and fentanyl. We make one 24 panel
Serenity Cup®: the 24 panel drug test cup with nitazene, alpha-PVP and medetomidine.
It also checks the sample for tampering with 3 adulterant pads. It is labeled Forensic Use Only.
Price: $4.19 per cup for a single cup or 25 cups, $3.59 at 1,000 and $2.79 at 20,000+.

How to Use a 24 Panel Drug Test Cup?
Step 1 – Collect
Open the sealed pouch just before use. The donor fills the cup to the line. Close the lid until it clicks.
Step 2 – Check temperature
Within 2 to 4 minutes, look for the green dot on the temperature strip. A reading outside the range can mean a cooled or substituted sample; recollect per your policy.
Step 3 – Read
Peel off the label and read at 5 minutes. Do not read after 8 minutes.
Positive (preliminary)
Only the control line (C) shows for a drug. Confirm by GC-MS or LC-MS/MS before acting.
Negative
Lines at C and T. A faint T line still counts as negative.
Invalid
No C line. Retest with a new cup. If it happens again, call 561-897-9238 with the lot number. See our guide to reading a drug test cup.
What Our 24 Panel Cup Does Not Test
- › Barbiturates, tramadol and tricyclic antidepressants. None of these is on our 24 panel list.
- › Tianeptine (ZaZa) and Delta-8 THC. If you need those, compare our 22 panel cup or our 28 panel drug test cups.
Why a 24 Panel Cup Adds Xylazine, Medetomidine and Fentanyl
Most 5 to 12 panel cups miss the drugs now showing up in the street supply. Fentanyl and other synthetic opioids don't reliably trigger an opiate strip, so they need their own strips. Xylazine and medetomidine are veterinary sedatives sold mixed into opioids. Read more about xylazine, ketamine, gabapentin in urine, kratom in urine and EtG in urine.
Prescribed drugs, such as gabapentin, buprenorphine or oxycodone, can explain a positive. Record current medicines at collection.
Who Uses a 24 Panel Drug Test Cup?
| Setting | Why a 24 panel cup fits |
|---|---|
| Probation and parole | One screen covers street drugs, fentanyl, xylazine and alcohol |
| Drug courts | Broad screen for phase checks; send non-negatives to a lab before using them as evidence |
| Corrections and jails | Catches K2, synthetic stimulants and sedatives that small panels miss |
| Treatment and sober living | Shows BUP and MTD alongside relapse drugs and EtG |
Not for DOT tests: those need an HHS-certified lab (49 CFR Part 40). For clinics, use a CLIA-waived drug test.

When to Choose a 24 Panel Cup?
Choose a 24 panel cup when your program must screen for fentanyl and the drugs mixed with it, plus alcohol, K2 and sample tampering, in one collection. Choose the Ultra cup to separate heroin (6-MAM) from other opiates. Choose the nitazene cup to cover nitazenes and medetomidine. If you don't need those drugs, a 22 or 24 panel cup costs less per test.
The 500 ng/mL cutoff is calibrated to minimize false positives from incidental exposure — such as hand sanitizer or mouthwash — while reliably catching actual drinking. At this level, approximately 76% of heavy drinking episodes are detected within one day, with a false positive rate of only about 3%.
For programs requiring abstinence verification where daily testing is impractical, EtG is the difference between knowing and guessing.
24 Panel Drug Test Cup — Frequently Asked Questions
An 18 panel drug test detects eighteen different drug categories including opioids, stimulants, benzodiazepines, marijuana, fentanyl, alcohol metabolites, prescription medications commonly associated with misuse or abuse, and additional expanded substances depending on the test configuration.
The test commonly includes AMP, BAR, BZO, BUP, COC, MDMA, ETG, FEN, GAB, KET, THC, MTD, MET, OPI, OXY, PCP, TRA, and one additional expanded panel analyte depending on manufacturer configuration
Yes. Expanded 18 panel drug tests commonly include a dedicated fentanyl (FEN) strip because fentanyl is not reliably detected on many older or lower panel opiate tests.
Yes. Panels containing ETG (Ethyl Glucuronide) can detect alcohol metabolites in urine for up to several days after alcohol consumption depending on usage patterns.
Most 18 panel cups use immunoassay screening technology designed for rapid preliminary detection with high accuracy when used properly. Non-negative results may require laboratory confirmation using GC/MS or LC/MS testing
ETG stands for Ethyl Glucuronide, a metabolite produced after alcohol consumption. ETG testing is used to identify recent alcohol exposure.
Cutoff levels are predefined concentration thresholds that determine whether a sample is reported as negative or preliminary positive during screening
Yes. Excessive fluid intake may dilute urine samples. Many test cups include adulteration or specimen validity checks to help identify dilution or tampering attempts.
Specimen validity testing helps verify whether a urine sample is authentic by checking temperature, creatinine, pH, and specific gravity levels
Yes. Expanded 18 panel configurations commonly include ketamine (KET), particularly in forensic, rehabilitation, and probation testing environments
An 18 panel drug test includes additional modern substances such as fentanyl, ETG alcohol testing, gabapentin, ketamine, tramadol, and other expanded analytes that are often excluded from smaller panel tests.
Detection windows vary by substance, metabolism, frequency of use, and hydration levels. Some drugs may only be detectable for 1–3 days, while others like THC or PCP may remain detectable longer.
Yes. Probation and court-ordered testing programs often use expanded panel tests to monitor a broader range of illicit drugs, prescription medications, and alcohol-related substances.
Yes. Legitimately prescribed medications such as benzodiazepines, oxycodone, methadone, buprenorphine, tramadol, or other included substances may trigger positive results if those substances are part of the panel.
Yes. THC panels screen for marijuana metabolites commonly associated with cannabis use.
Most rapid urine drug test cups provide preliminary results within approximately 5 minutes after sample collection.
A non-negative or preliminary positive result may be sent to a certified laboratory for confirmatory testing before final reporting
Yes. Many organizations upgraded to expanded panel testing specifically to improve fentanyl detection due to the increased prevalence of synthetic opioid misuse and overdose cases
Expanded panels improve detection coverage for newer or increasingly abused substances such as fentanyl, ketamine, tramadol, gabapentin, and additional modern analytes compared to traditional drug tests
