28 Panel Drug Test Cups
A 28 panel drug test cup screens one urine sample for 27 drug strips plus a 3-in-1 adulterant check,
with results in about 5 minutes. It is the largest instant cup we make, built for programs that must
catch fentanyl, xylazine, K2 and alcohol on one screen. We make two 28 panel Serenity Cup® versions.
Both share 24 drug strips and the adulterant check. The Ultra 28 panel drug test cup adds 6-MAM (heroin),
alpha-PVP (flakka) and a second THC strip at 25 ng/mL. The newer 28 panel cup with nitazene and
medetomidine adds nitazene, medetomidine and delta-8 THC instead. Both are labeled Forensic Use Only.
Both cost $4.49 per cup at 25 cups, down to $3.09 per cup at 20,000+.

A cutoff is the concentration at or above which a strip reads positive. Our guide to how cutoff levels work explains why the same drug can read differently on different tests.
How to Use a 28 Panel Drug Test Cup?
Step 1 – Collect
Open the sealed pouch just before use. The donor fills the cup to the line. Close the lid until it clicks.
Step 2 – Check temperature
At 2–4 minutes, check where the green dot appears on the temperature strip. It should show 90–100°F. A reading outside that range can mean a cooled or substituted sample; recollect per your policy.
Step 3 – Read
Peel off the label and read at 5 minutes. Do not read after 8 minutes. Compare the adulterant pads to the color chart in the kit.
Positive (preliminary)
Only the control line (C) shows for a drug. The drug is at or above the cutoff. Confirm by GC-MS or LC-MS/MS before acting.
Negative
Lines at C and T. A faint T line still counts as negative.
Invalid
No C line. Retest with a new cup. If it happens again, call 561-897-9238 with the lot number. For photos of each result, see our step-by-step guide to reading a drug test cup. To read the specific gravity, pH and creatinine pads, see what adulterants are in drug testing.
What a 28 Panel Cup Covers, Drug Class by Drug Class?
There is no single standard 28 panel. Each maker picks its own set, and the newer cups swap older strips for drugs now found in the street supply. Our two cups split the same way. Check the list your court, program or agency requires before you order. For a longer explanation of each drug, read our 28 panel drug test guide.
-
Opioids and what's mixed with them:
Opiates (OPI 300), oxycodone, methadone, buprenorphine, tramadol and fentanyl on both cups. Both also test xylazine, a veterinary sedative mixed into fentanyl (how to test for xylazine), and tianeptine, sold as "ZaZa" (what ZaZa is). The Ultra adds 6-MAM, a marker of heroin specifically (6-MAM strip). The nitazene cup adds nitazenes (nitazenes explained) and medetomidine.
-
Stimulants:
Amphetamines, methamphetamine, cocaine, MDMA and methylphenidate (Ritalin on a drug test) on both. The Ultra adds alpha-PVP, known as flakka.
-
Hallucinogens and dissociatives:
LSD (does LSD show up in urine), psilocybin, PCP and ketamine on both cups.
-
Sedatives and prescription drugs:
Benzodiazepines, barbiturates and tricyclic antidepressants on both. Neither cup tests gabapentin.
-
Cannabis, kratom and alcohol:
THC at 50 ng/mL, K2/Spice (K2 strip) and kratom (kratom in urine) on both. The Ultra adds a THC 25 strip. The nitazene cup adds delta-8 THC (does delta-8 test positive for THC). EtG at 300 ng/mL on both shows recent drinking, not current intoxication (how long EtG stays in urine).
-
Sample checks:
Specific gravity, pH and creatinine on both, plus the temperature strip.
Both cups are labeled Forensic Use Only.
Built for Collection Sites
Each strip has a control line, so you can see that the test ran. The temperature strip and the adulterant pads check the sample at collection, and the double click-top lid keeps it sealed for storage or for shipping a non-negative to a lab. The cup is self-contained, so no other equipment is needed.

Can Medicines Cause a Positive on a 28 Panel Cup?
Yes. A 28 panel cup includes several prescription drugs, so a valid prescription can explain a positive. Examples are buprenorphine, methadone, oxycodone, tramadol, benzodiazepines, methylphenidate and tricyclic antidepressants.
Record current medicines at collection, confirm positives by GC-MS or LC-MS/MS, and have a medical review officer review them where your program requires it. Cross-reacting substances for each strip are listed in the package insert.

When to Choose a 28 Panel Cup?
Choose a 28 panel cup when your program must screen for fentanyl and the drugs mixed with it, plus alcohol, K2 and sample tampering, in one collection. Choose the Ultra cup to separate heroin (6-MAM) from other opiates. Choose the nitazene cup to cover nitazenes and medetomidine. If you don't need those drugs, a 22 or 24 panel cup costs less per test.
The 500 ng/mL cutoff is calibrated to minimize false positives from incidental exposure — such as hand sanitizer or mouthwash — while reliably catching actual drinking. At this level, approximately 76% of heavy drinking episodes are detected within one day, with a false positive rate of only about 3%.
For programs requiring abstinence verification where daily testing is impractical, EtG is the difference between knowing and guessing.
Where 28 Panel Cups Are Used
The table below matches common forensic settings with the cup that fits best.
| Setting | Main uses | Best-fit cup | Why |
|---|---|---|---|
| Probation and parole | Random and compliance testing | Either cup | Fentanyl, xylazine, alcohol and adulterant checks in one screen |
| Drug courts | Phase progression, relapse checks | Either cup | Send non-negatives to a lab before using them as evidence |
| Corrections | Intake and post-visit testing | Nitazene cup | Covers nitazenes, medetomidine and K2 |
| Treatment and sober living (forensic or compliance programs) | Relapse, drinking, treatment adherence | Ultra cup | 6-MAM separates heroin from prescribed opiates; BUP and MTD show treatment drugs |
| Employers (non-DOT) | Pre-employment, random | DOT-regulated tests must go to an HHS-certified lab (49 CFR Part 40) |

Built for Testing Administrators in Law Enforcement Settings
A 28 panel drug test cup integrates directly into established law enforcement and government screening procedures without requiring additional equipment or extensive retraining. This allows agencies to maintain consistent testing protocols across departments and jurisdictions.
It is particularly useful in:
- Probation and parole monitoring programs.
- Court-ordered compliance testing.
- Correctional intake and supervision workflows.
- Government-administered substance screening initiatives.
Advanced Control for Program Managers
Balancing detection capability with fiscal responsibility? That's why we price these fairly.
A 28 panel configuration supports program oversight by expanding detection capabilities for controlled substance monitoring programs managed by government agencies ensuring:
- Broader substance detection coverage for compliance monitoring
- Enhanced visibility into emerging substance use patterns relevant to public safety programs
- Support for structured supervision programs under legal mandate
- Suitability for forensic-level screening environments
- This helps agencies maintain stronger oversight within regulated populations
28 Panel Drug Test Cup — Frequently Asked Questions
An 18 panel drug test detects eighteen different drug categories including opioids, stimulants, benzodiazepines, marijuana, fentanyl, alcohol metabolites, prescription medications commonly associated with misuse or abuse, and additional expanded substances depending on the test configuration.
The test commonly includes AMP, BAR, BZO, BUP, COC, MDMA, ETG, FEN, GAB, KET, THC, MTD, MET, OPI, OXY, PCP, TRA, and one additional expanded panel analyte depending on manufacturer configuration
Yes. Expanded 18 panel drug tests commonly include a dedicated fentanyl (FEN) strip because fentanyl is not reliably detected on many older or lower panel opiate tests.
Yes. Panels containing ETG (Ethyl Glucuronide) can detect alcohol metabolites in urine for up to several days after alcohol consumption depending on usage patterns.
Most 18 panel cups use immunoassay screening technology designed for rapid preliminary detection with high accuracy when used properly. Non-negative results may require laboratory confirmation using GC/MS or LC/MS testing
ETG stands for Ethyl Glucuronide, a metabolite produced after alcohol consumption. ETG testing is used to identify recent alcohol exposure.
Cutoff levels are predefined concentration thresholds that determine whether a sample is reported as negative or preliminary positive during screening
Yes. Excessive fluid intake may dilute urine samples. Many test cups include adulteration or specimen validity checks to help identify dilution or tampering attempts.
Specimen validity testing helps verify whether a urine sample is authentic by checking temperature, creatinine, pH, and specific gravity levels
Yes. Expanded 18 panel configurations commonly include ketamine (KET), particularly in forensic, rehabilitation, and probation testing environments
An 18 panel drug test includes additional modern substances such as fentanyl, ETG alcohol testing, gabapentin, ketamine, tramadol, and other expanded analytes that are often excluded from smaller panel tests.
Detection windows vary by substance, metabolism, frequency of use, and hydration levels. Some drugs may only be detectable for 1–3 days, while others like THC or PCP may remain detectable longer.
Yes. Probation and court-ordered testing programs often use expanded panel tests to monitor a broader range of illicit drugs, prescription medications, and alcohol-related substances.
Yes. Legitimately prescribed medications such as benzodiazepines, oxycodone, methadone, buprenorphine, tramadol, or other included substances may trigger positive results if those substances are part of the panel.
Yes. THC panels screen for marijuana metabolites commonly associated with cannabis use.
Most rapid urine drug test cups provide preliminary results within approximately 5 minutes after sample collection.
A non-negative or preliminary positive result may be sent to a certified laboratory for confirmatory testing before final reporting
Yes. Many organizations upgraded to expanded panel testing specifically to improve fentanyl detection due to the increased prevalence of synthetic opioid misuse and overdose cases
Expanded panels improve detection coverage for newer or increasingly abused substances such as fentanyl, ketamine, tramadol, gabapentin, and additional modern analytes compared to traditional drug tests
