18 Panel Drug Test Cups
An 18 panel drug test cup screens one urine sample for eighteen drugs, with results in about 5 minutes.
It starts from the classic 12 panel drug test and adds fentanyl, alcohol (EtG), ketamine and tramadol,
plus two more strips that change by version. We make six 18 panel Serenity Cup® versions. All six test
the same 16 drugs. The other two strips, and the extras, are the difference: kratom, K2, xylazine, tricyclic
antidepressants (TCA), gabapentin, or pH, specific gravity and creatinine checks. Four cups hold 120 mL
and two are 90 mL economy cups. All are labeled Forensic Use Only. Prices run from $3.29 to $3.49 per cup
at 25 cups, down to $1.89 to $2.09 at 20,000+.

A cutoff is the concentration at or above which a strip reads positive. Our guide to how cutoff levels work explains why the same drug can read differently on different tests.
How to Use an 18 Panel Drug Test Cup
Step 1 – Collect
Open the sealed pouch just before use. The donor fills the cup to the line. Close the lid until it clicks.
Step 2 – Check temperature
Within 4 minutes, check that the temperature strip shows a reading between 90 and 100°F. A reading outside that range can mean a cooled or substituted sample; recollect per your policy.
Step 3 – Read
Peel off the label and read at 5 minutes. Do not read after the time limit in the insert.
Positive (preliminary)
Only the control line (C) shows for a drug. The drug is at or above the cutoff. Confirm by GC-MS or LC-MS/MS before acting.
Negative
Lines at C and T. A faint T line still counts as negative.
Invalid
No C line. Retest with a new cup. If it happens again, call 561-897-9238 with the lot number. For photos of each result, see our step-by-step guide to reading a drug test cup.
What an 18 Panel Cup Covers, Drug Class by Drug Class?
There is no single standard 18 panel. Each maker picks its own eighteen. Many retail cups add TCA, K2 and kratom to EtG, fentanyl and tramadol, and leave out ketamine. All of our 18 panel cups test ketamine. Check the list your court, program or employer requires before you order.
-
Opioids:
Opiates (OPI 300), oxycodone, methadone, buprenorphine, fentanyl and tramadol on every cup. Fentanyl and tramadol are synthetic, so opiate strips don't reliably catch them. That's why they get their own strips. More on what the tramadol strip screens for.
-
Stimulants and party drugs:
Amphetamines, methamphetamine, cocaine, MDMA, PCP and ketamine on every cup (ketamine facts).
-
Sedatives and prescription drugs:
Benzodiazepines and barbiturates on every cup. Gabapentin on one cup (how long gabapentin stays in urine); tricyclic antidepressants on one cup (what a TCA test screens for).
-
Cannabis, K2 and kratom:
Marijuana (THC 50) on every cup. K2 (synthetic cannabinoids) on the 90 mL K2 cup. Kratom on five of the six cups (kratom in urine).
-
Xylazine:
A veterinary sedative found mixed into street fentanyl. Two cups have a xylazine strip at 1000 ng/mL.
-
Alcohol:
EtG at 300 ng/mL on every cup. EtG is a breakdown product of alcohol that shows recent drinking, not current intoxication. See how long EtG stays in urine.
-
Sample checks:
Three cups add pH, specific gravity and creatinine pads. Read what adulterants are and how cups catch them.
All six cups are labeled Forensic Use Only.
Built for Collection Sites
Each strip has a control line, so you can see that the test ran. A built-in temperature strip checks the sample at collection, and the lid keeps it sealed for storage or for shipping a non-negative to a lab. The cup is self-contained, so no other equipment is needed.

How the Test Strips Work
Each strip is a lateral flow immunoassay. If the drug is in the urine above the cutoff, it blocks the antibody from binding at the test zone, so no T line appears (preliminary positive). If the drug is absent or below the cutoff, the T line appears (negative). The result is qualitative: it shows above or below the cutoff, not how much was taken.

What Can Cause a False Positive?
A screening strip reacts to a drug class, so some legal substances can cross-react. Common examples are cold medicines with pseudoephedrine (amphetamine strip), poppy seeds (opiate strip) and some antidepressants (TCA strip). Prescribed tramadol, oxycodone, buprenorphine, methadone, benzodiazepines or gabapentin will also screen positive. Record current medicines at collection, confirm positives by GC-MS or LC-MS/MS, and have a medical review officer review results where your program requires it.
The 500 ng/mL cutoff is calibrated to minimize false positives from incidental exposure — such as hand sanitizer or mouthwash — while reliably catching actual drinking. At this level, approximately 76% of heavy drinking episodes are detected within one day, with a false positive rate of only about 3%.
For programs requiring abstinence verification where daily testing is impractical, EtG is the difference between knowing and guessing.
Where 18 Panel Cups Are Used?
| Setting | Main uses | Best-fit cup | Why |
|---|---|---|---|
| Probation and parole | Random and compliance testing | 90 mL K2 cup with adulterants | Lower cost per test; checks for diluted samples |
| Drug courts | Phase progression, relapse checks | Kratom cup with adulterants | Send non-negatives to a lab before using them as evidence |
| Treatment, MAT and sober living | BUP/MTD adherence, relapse, drinking | Gabapentin and kratom cup | Shows treatment drugs alongside gabapentin, kratom, fentanyl and EtG |
| Harm reduction and fentanyl-focused programs | Fentanyl and xylazine exposure | Xylazine cup (120 or 90 mL) | Fentanyl and xylazine strips on the same screen |
| Corrections | Intake, post-visit testing | 90 mL K2 cup | Synthetic cannabinoids plus sample checks |
| Employers (non-DOT) | Pre-employment, random | Any version | DOT-regulated tests must go to an HHS-certified lab (49 CFR Part 40) |

When an 18 Panel Cup Makes Sense
Pick an 18 panel cup when your program must watch fentanyl, alcohol, ketamine and tramadol along with the usual street drugs, and you need one more pair: kratom, K2, xylazine, TCA or gabapentin. One cup replaces a 12 panel cup plus separate EtG and fentanyl tests, so there is one collection, one read and one record. If you don't need the extras, a 17 panel cup costs less. If you need 6-MAM, step up to 20 panels.
What Does “Forensic Use Only” Mean?
- › The cup has not been cleared by the FDA through 510(k) for clinical diagnosis.
- › It is sold for forensic and compliance testing: courts, probation and parole, corrections, law enforcement, sober living and the labs that test for them.
- › It is not for diagnosis, treatment decisions, home use or resale as an over-the-counter test.
- › Results are preliminary. Many courts and programs require lab confirmation before a result is used.

18 Panel Drug Test Cup — Frequently Asked Questions
An 18 panel drug test detects eighteen different drug categories including opioids, stimulants, benzodiazepines, marijuana, fentanyl, alcohol metabolites, prescription medications commonly associated with misuse or abuse, and additional expanded substances depending on the test configuration.
The test commonly includes AMP, BAR, BZO, BUP, COC, MDMA, ETG, FEN, GAB, KET, THC, MTD, MET, OPI, OXY, PCP, TRA, and one additional expanded panel analyte depending on manufacturer configuration
Yes. Expanded 18 panel drug tests commonly include a dedicated fentanyl (FEN) strip because fentanyl is not reliably detected on many older or lower panel opiate tests.
Yes. Panels containing ETG (Ethyl Glucuronide) can detect alcohol metabolites in urine for up to several days after alcohol consumption depending on usage patterns.
Most 18 panel cups use immunoassay screening technology designed for rapid preliminary detection with high accuracy when used properly. Non-negative results may require laboratory confirmation using GC/MS or LC/MS testing
ETG stands for Ethyl Glucuronide, a metabolite produced after alcohol consumption. ETG testing is used to identify recent alcohol exposure.
Cutoff levels are predefined concentration thresholds that determine whether a sample is reported as negative or preliminary positive during screening
Yes. Excessive fluid intake may dilute urine samples. Many test cups include adulteration or specimen validity checks to help identify dilution or tampering attempts.
Specimen validity testing helps verify whether a urine sample is authentic by checking temperature, creatinine, pH, and specific gravity levels
Yes. Expanded 18 panel configurations commonly include ketamine (KET), particularly in forensic, rehabilitation, and probation testing environments
An 18 panel drug test includes additional modern substances such as fentanyl, ETG alcohol testing, gabapentin, ketamine, tramadol, and other expanded analytes that are often excluded from smaller panel tests.
Detection windows vary by substance, metabolism, frequency of use, and hydration levels. Some drugs may only be detectable for 1–3 days, while others like THC or PCP may remain detectable longer.
Yes. Probation and court-ordered testing programs often use expanded panel tests to monitor a broader range of illicit drugs, prescription medications, and alcohol-related substances.
Yes. Legitimately prescribed medications such as benzodiazepines, oxycodone, methadone, buprenorphine, tramadol, or other included substances may trigger positive results if those substances are part of the panel.
Yes. THC panels screen for marijuana metabolites commonly associated with cannabis use.
Most rapid urine drug test cups provide preliminary results within approximately 5 minutes after sample collection.
A non-negative or preliminary positive result may be sent to a certified laboratory for confirmatory testing before final reporting
Yes. Many organizations upgraded to expanded panel testing specifically to improve fentanyl detection due to the increased prevalence of synthetic opioid misuse and overdose cases
Expanded panels improve detection coverage for newer or increasingly abused substances such as fentanyl, ketamine, tramadol, gabapentin, and additional modern analytes compared to traditional drug tests
