21 Panel Drug Test Cup
A 21 panel drug test cup screens one urine sample for up to twenty-one drugs, with results in about 5 minutes.
It builds on the classic 12 panel drug test by adding fentanyl, alcohol (EtG), xylazine, kratom, K2, psilocybin and
more. Every cup here also checks the sample for tampering. We make three 21 panel Serenity Cup® versions.
All three share 16 drug strips and three adulterant checks. The psilocybin and K2 cup adds barbiturates,
MDMA, ketamine and tramadol. The ZAZA (tianeptine) cup adds the same four plus tianeptine. The DXM, gabapentin
and LSD cup adds dextromethorphan, gabapentin, LSD, 6-MAM and tianeptine, with lower cutoffs on five strips.
All three are labeled Forensic Use Only and cost the same: $3.99 per cup at 25, down to $2.59 per cup at 20,000+.

Numbers are cutoffs in ng/mL. A cutoff is the concentration at or above which a strip reads positive. A lower cutoff picks up smaller amounts. Our guide to how cutoff levels work explains why.
How to Use a 21 Panel Drug Test Cup?
1 – Collect
Open the sealed pouch just before use. The donor fills the cup to the line. Close the lid until it clicks.
2 – Check temperature
Within 4 minutes, check that the temperature strip reads between 90 and 100°F. A reading outside that range can mean a cooled or substituted sample; recollect per your policy.
3 – Read
Peel off the label and read the drug strips and the adulterant pads at 5 minutes. Do not read after 8 minutes.
Positive (preliminary)
Only the C line shows for that drug. The drug is at or above the cutoff. Confirm by GC-MS or LC-MS/MS before acting.
Negative
Two lines, one at C and one at T. A faint T line still counts as negative.
Invalid
No C line. Retest with a new cup. If it happens again, call 561-897-9238 with the lot number.
What a 21 Panel Cup Covers, Drug Class by Drug Class?
There is no single standard 21 panel. Each lab and cup maker picks its own list. Some lab panels, such as Labcorp's ToxAssure Flex 21, are confirmation tests run in a lab, not instant cups. Our three cups cover these classes:
-
Opioids:
Opiates, oxycodone, methadone, buprenorphine and fentanyl on all three cups; tramadol on two; 6-MAM, a heroin-only marker, on the DXM cup. Fentanyl and tramadol are synthetic, so opiate strips don't reliably catch them. More on what the tramadol strip screens for.
-
Xylazine and tianeptine:
Xylazine on all three cups; tianeptine (ZAZA) on two. Xylazine is a veterinary sedative found mixed into street opioids; read why xylazine is a real threat. Tianeptine is sold in some gas stations as "ZAZA"; see tianeptine's long-term effects.
-
Stimulants and party drugs:
Amphetamines, methamphetamine and cocaine on all three; MDMA and ketamine on two (ketamine facts); PCP on all three.
-
Hallucinogens:
Psilocybin on all three cups; LSD and DXM on the DXM cup.
-
Sedatives and prescription drugs:
Benzodiazepines on all three; barbiturates on two; gabapentin on the DXM cup.
-
Cannabis, K2, kratom and alcohol:
THC 50, synthetic marijuana (K2) and kratom on all three. EtG on all three, at 300 or 500 ng/mL. EtG is a breakdown product of alcohol that shows recent drinking, not current intoxication. See how long EtG stays in urine.
-
What these cups don't test:
Tricyclic antidepressants, nitazenes, methylphenidate, GHB, MDPV and cotinine. For nitazenes, see our 22 panel cup below.
All three cups are labeled Forensic Use Only.
Built for Collection Sites
Each strip has a control line, so you can see that the test ran. The temperature strip and three adulterant pads check the sample at collection. The click-top lid keeps it sealed for storage or for shipping a non-negative to a lab. The cup is self-contained, so no other equipment is needed.

Why a 21 Panel Cup Includes Xylazine, Tianeptine and Psilocybin
Most 5 to 12 panel cups don't test for these drugs, but some supervision programs now ask for them. Xylazine is not an opioid, so opiate and fentanyl strips don't detect it; it needs its own strip. Tianeptine is an antidepressant in some countries but is not approved in the US.
A prescription can explain some positives, such as gabapentin, tramadol, oxycodone or buprenorphine. Record current medicines at collection, and have a medical review officer review positives where your program requires it.

Why 21 Panel Drug Test Cups Include Emerging Drugs Like Fentanyl and Xylazine
Modern 21 panel drug test cups increasingly include synthetic and emerging substances due to shifts in global drug supply trends. Substances such as fentanyl and xylazine have become critical inclusions because of their rising involvement in overdose cases and drug adulteration.
Fentanyl is a highly potent synthetic opioid often mixed with other street drugs, making detection essential in emergency toxicology screening.
Xylazine, a veterinary sedative not intended for human use, has been increasingly identified in opioid mixtures, creating new challenges for standard drug testing systems.
Including these substances in a 21 panel drug test cup reflects an adaptation to current drug markets rather than traditional pharmaceutical-only screening models. A 21 panel drug screen test will show these results thus ensuring a clean screen test.
Where 21 Panel Cups Are Used?
The table matches common forensic settings to the cup that fits them best.
| Setting | Main uses | Best-fit cup | Why |
|---|---|---|---|
| Probation and parole | Random and compliance testing | PSY/K2 or ZAZA cup | Opioids, xylazine, party drugs and alcohol on one screen, with tamper checks |
| Drug courts | Phase progression, relapse checks | Any of the three | Send non-negatives to a lab before using them as evidence |
| Jails, prisons and juvenile detention | Intake and random testing | ZAZA cup | Covers tianeptine and K2 along with the standard drugs |
| Forensic labs | Pre-screen before confirmation | DXM/GAB/LSD cup | Lower cutoffs on five strips plus 6-MAM for heroin |
| Treatment and sober living programs | Relapse and drinking checks | DXM/GAB/LSD cup | Gabapentin, buprenorphine, methadone and EtG on one cup |

When a 21 Panel Cup Makes Sense
Pick a 21 panel cup when your program has to watch xylazine, kratom, K2 or psilocybin at the same time as fentanyl, alcohol and the usual street drugs. One cup replaces a 12 panel cup plus several single-drug tests, so there is one collection, one read and one record. If you don't need those drugs, a smaller panel costs less per test.
Still deciding? Read why you might need a 21 panel drug test.
21 Panel Drug Test Cup – Real-World Use Cases and Why Demand Has Increased in Modern Drug Screening
One of the most common real-world applications is workplace safety and compliance testing, particularly in industries such as construction, transport, logistics, and healthcare. Employers use extended panel drug testing when roles involve machinery operation, public safety responsibility, or controlled environments where impairment risk must be minimize. In clinical and rehabilitation settings, 21 panel drug test cups are used to monitor patients undergoing substance misuse treatment or long-term prescription therapy. Another growing use case is forensic and probation-related screening, where authorities require wider substance coverage due to the rise of synthetic drugs in street-level drug mixtures. Substances like xylazine, often found mixed with opioids in illicit supplies, have created new challenges for traditional drug testing systems.

What Does "Forensic Use Only" Mean?
All three 21 panel cups are labeled Forensic Use Only. That means:
- The cup has not been cleared by the FDA through 510(k) for clinical diagnosis.
- It is sold for forensic testing: courts, police, probation and parole, juvenile detention, jails and prisons, government agencies and forensic labs.
- It is not for diagnosis, treatment decisions, home use or resale as an over-the-counter test.
- Results are preliminary. Many courts and programs require lab confirmation before a result is used.

21 Panel Drug Test Cup Frequently Asked Questions
A 21 panel drug test detects twenty different drug categories including opioids, stimulants, benzodiazepines, marijuana, fentanyl, alcohol metabolites, prescription medications commonly associated with misuse or abuse, and additional expanded substances used in forensic and clinical testing.
The test commonly includes AMP, BAR, BZO, BUP, COC, MDMA, ETG, FEN, GAB, KET, THC, MTD, MET, OPI, OXY, PCP, TRA, and additional expanded analytes depending on manufacturer configuration
Yes. Expanded 21 panel drug tests commonly include a dedicated fentanyl (FEN) strip because fentanyl is not reliably detected on many older or lower panel opiate tests.
Yes. Panels containing ETG (Ethyl Glucuronide) can detect alcohol metabolites in urine for up to several days after alcohol consumption depending on usage patterns.
Most 21 panel cups use immunoassay screening technology designed for rapid preliminary detection with high accuracy when used properly. Non-negative results may require laboratory confirmation using GC/MS or LC/MS testing.
ETG stands for Ethyl Glucuronide, a metabolite produced after alcohol consumption. ETG testing is used to identify recent alcohol exposure.
Yes. Excessive fluid intake may dilute urine samples. Many test cups include adulteration or specimen validity checks to help identify dilution or tampering attempts.
Yes. Expanded 21 panel configurations commonly include ketamine (KET), particularly in forensic, rehabilitation, and probation testing environments.
A non-negative or preliminary positive result may be sent to a certified laboratory for confirmatory testing before final reporting.
Expanded panels improve detection coverage for newer or increasingly abused substances such as fentanyl, ketamine, tramadol, gabapentin, and additional modern analytes compared to traditional drug tests.
A 21 panel drug test includes additional modern substances such as fentanyl, ETG alcohol testing, gabapentin, ketamine, tramadol, and other expanded analytes that are often excluded from smaller panel tests
Detection windows vary by substance, metabolism, frequency of use, and hydration levels. Some drugs may only be detectable for 1–3 days, while others like THC or PCP may remain detectable longer
Yes. Probation and court-ordered testing programs often use expanded panel tests to monitor a broader range of illicit drugs, prescription medications, and alcohol-related substances.
Yes. Legitimately prescribed medications such as benzodiazepines, oxycodone, methadone, buprenorphine, tramadol, or other included substances may trigger positive results if those substances are part of the panel.
Yes. THC panels screen for marijuana metabolites commonly associated with cannabis use.
Cutoff levels are predefined concentration thresholds that determine whether a sample is reported as negative or preliminary positive during screening.
Specimen validity testing helps verify whether a urine sample is authentic by checking temperature, creatinine, pH, and specific gravity levels.
Most rapid urine drug test cups provide preliminary results within approximately 5 minutes after sample collection.
Yes. Many organizations upgraded to expanded panel testing specifically to improve fentanyl detection due to the increased prevalence of synthetic opioid misuse and overdose cases.
These tests are commonly used by employers, rehabilitation centers, probation departments, pain management clinics, hospitals, forensic programs, and occupational health providers.
