What Are Opiates?

Opiates are drugs that come from the opium poppy, such as morphine and codeine. Heroin is made from morphine and is quickly converted back to morphine in the body, so opiate tests detect it as morphine. Semi-synthetic and synthetic opioids – such as oxycodone, fentanyl and methadone – are chemically different and need their own tests.

Why Test for Opiates?

Opiate screening is part of most standard workplace, clinical and treatment-program drug panels. The 2000 ng/mL cutoff is the federal (SAMHSA) workplace standard and reduces positives caused by eating poppy seeds. Programs that need higher sensitivity often use a 300 ng/mL opiate test instead.

How to Use the Opiate Dip Card

Remove the dip card from the sealed foil pouch and take off the cap.
Dip the exposed test strip into the urine sample for 10–15 seconds.
Allow the urine to migrate up the test strip.
Replace the cap and lay the dip card flat on a clean, dry, non-absorbent surface.
Read the results at 5 minutes. Do not read after 10 minutes.

How to Read Opiate Dip Card Results

Positive

Only one colored line appears, in the control region (C). No line appears in the test region (T). This means morphine was detected at or above 2000 ng/mL.

Negative

Two colored lines appear: one in the control region (C) and one in the test region (T). Any line in the T region, even a very faint one, counts as negative.

Invalid

No line appears in the control region (C). Discard the result and repeat the test with a new dip card. If the problem persists, stop using the lot and contact 12 Panel Now.

Opiate Dip Card – Frequently Asked Questions

What is the difference between an OPI 2000 and an OPI 300 test?

The number is the cutoff in ng/mL. 2000 ng/mL is the federal workplace cutoff – it was raised from 300 to 2000 ng/mL in December 1998 to avoid positives from poppy seeds. 300 ng/mL is more sensitive and is often preferred in clinical and treatment settings where low-level use matters.

Can poppy seeds cause a positive opiate test?

It is much less likely at 2000 ng/mL than at 300 ng/mL. Most people who eat poppy seeds stay below 2000 ng/mL, but eating large amounts can occasionally push morphine above the cutoff for up to about 48 hours. Confirm unexpected positives with a laboratory test.

Does the OPI test detect oxycodone, fentanyl or methadone?

Not reliably. Opiate tests are designed to detect morphine and codeine. Oxycodone is usually not detected, and fentanyl and methadone are not detected. Use a dedicated OXY dip card, a fentanyl test or an EDDP (methadone) dip card for those drugs.

Will this test detect heroin?

Yes, indirectly. Heroin is rapidly converted to morphine in the body, and morphine is what this test detects. A positive result cannot tell heroin use apart from morphine or codeine use – a 6-MAM test or laboratory confirmation is needed for that.

How long do opiates stay in urine?

Morphine and codeine are usually detectable for up to about 3 days after use. After heroin use, morphine can typically be detected for 2–4 days. Detection time varies with dose, frequency of use and metabolism.

Will prescription codeine or morphine cause a positive result?

Yes. Prescription medications that contain codeine (such as some cough syrups and Tylenol® with codeine) or morphine can produce a positive result. Verify prescriptions before interpreting results.

How do I read a very faint line?

Any visible line in the test region (T), even a very faint one, means the result is negative. The result is positive only when a line appears in the control region (C) and no line appears in the test region (T).

Is a positive result final?

No. This is a rapid screening test that gives a preliminary result. Confirm non-negative results with a laboratory test such as GC/MS or LC-MS/MS before making treatment, employment or legal decisions.
